- Enter the contact information for two contacts at your agency.
CLICK: Add a Row in the Seattle Operational Contacts section

ENTER: the contact’s name, phone number, and email address and select the contact’s role
CLICK: Submit
- REPEAT: steps 6 and 7 for the second contact

CONFIRM: or add the address of your office in Seattle (if applicable)

CONFIRM: or add your City Business License information (if applicable)
CLICK: Continue Application

CLICK: the checkbox to confirm you comply with the certification
